Our community-based services delay or avert more costly nursing home and institutional placement and, through Georgia’s Medicaid waiver programs, provide low income Georgians with the opportunity to receive quality personal care home and host home placement.  Altrus Family Model, Group Model and DD Host Home Model homes accept clients who are on a variety of Medicaid waivers.  These include:

We also serve as a referral agency.  If you know of someone who may be eligible for any of these programs based on the criteria noted at the State of Georgia’s link here, we will be happy to refer to the program in your community for approval.  This is just another way Altrus strives to provide the utmost in customer service to ensure that disabled and elderly citizens in Georgia receive the best care available in the community setting.

Here is a brief summary of each of the waiver programs we support.


This waiver provides enhanced primary care case management program that serves frail elderly and disabled beneficiaries to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services. The goals of the SOURCE program are to:

  • Integrate primary care, specialty care and home based care to eliminate fragmentation;
  • Reduce emergency room use, hospital and nursing home admissions caused by preventable medical complications;
  • Stabilize social and lifestyle factors that affect compliance, health status and quality of life;
  • Insure that current gaps in Medicaid benefits for medical and supported living services are addressed so they do not negatively affect health outcomes and cost; and
  • Reduce the need for long-term institutional placement.


This  Medicaid waiver program provides community-based social, health and support services to eligible consumers as an alternative to institutional placement in a nursing facility.  Placement in a Georgia licensed personal care home, group or family model is an option of this program. The Georgia Department of Community Health’s (DCH) Division of Medicaid partners with the Division of Aging Services (DAS) within the Department of Human Services (DHS) for the operational management of the program.

This waiver allows persons with physical disabilities, including TBI, aged 21-64 who meet the nursing facility or hospital level of care to remain living at home and in the community. Services offered under this waiver include: service coordination, respite, specialized medical equipment and supplies, counseling and home modification.  More details on the Independent Care Waiver Program can be found at this link.

Comprehensive Waiver (COMP)

The Comprehensive Supports Waiver Program is a (federal) Medicaid Home and Community-Based Services (HCBS) waiver program authorized in Section (§) 1915(c) of the Social Security Act. The Centers for Medicare & Medicaid Services (CMS), a federal agency), approved and provides oversight of COMP. The State of Georgia, Department of Behavioral Health and Developmental Disabilities (DBHDD), Division of Developmental Disabilities (DDD) implements the COMP Program as approved by CMS.

By design, Home and Community-Based Services waivers, particularly the COMP, provide an array of services and supports that enable persons with developmental disabilities to live in the community and in their own/family home. These services and supports address the needs of the individual and complement and/or supplement the services available to participants through the Medicaid State plan and other federal, state and local public programs.

The Comprehensive Supports Waiver Program includes a variety of service delivery options from which individuals and families can choose the most appropriate method to meet their respective needs Altrus supports those on this waiver by providing DD Host Home living options for individuals.
